Cystic Disease of Pancreas


Abstract
It has been said that there are many differences in the clinical features such as their incidence, predominant age and symptoms, and their management between pseudocysts and neoplastic cysts of the pancreas.
This author analysed 15 patients with pancreatic pseudocysts and 5 patients with neoplastic cysts of pancreas who had previously undergone operative managenents at the department of surgery of Chonnam University Hospital during the 10-year period
from
1980 through 1989
@ES The results were as follows:
1) 60% of pancreatic psueocysts was found in the 4th and 5th decade, where-as all of neoplastic cysts were found in the young females in the 2nd and 3rd decade.
2) Etiologic factors in 67% of pancreatic pseudocysts were biliary disease, alcohol and trauma.
3) The main clinical symptoms of pseudocysts were abdominal pain(100%), weight loss(47%), general weakness and fatigue(33%), and nausea and womiting(13%) compared with vague postprandial abdominal discomfort(60%), general weakness and
fatigue(60%), and
epigastiric pain (40%) in neoplasitc cysts of pancreas, Palpable abdminal mass was noticed in 47% of the pancreatic pseudocysts compared with 80% in the neoplastic cysts.
4) Increased serum amylase was occurred in 67% and leukocytosis in 60% in the pancreatic pseudocysts, but all of neoplastic cysts did not show such findings.
5) Frequently developing sites were head and body(675) in the pseudocysts compared with tail and body (80%) in the neo;lastic cysts. The size was between 7 cm and 30 cm in the psudocysts and between 7 cm and 15 cm in the neoplastic cysts.
6) Only drainage procedure was done in 80% with pseudocysts, 2 cases of external drainage were recurred but 90% with 10 cases performed internal drainage had satisfactory results. In cases of neoplastic cysts, no recurrent cyst was occurred in
80%
with
patients performed distal pancreatectomy during 1~4 years postoperatively.
7) The treatment of pseudocysts was drainage procedure of external drainage or internal. The cyst recurrence developed in all 2 cases of external drainage, but 10 cases of internal drainage showed 90% of success.
8) The patients with neoplastic cyst underwent distal pancreatectomy in 80% with the results of no evidence of recurring cyst to 4 years postoperatively.
